Penile (cavernous) nerves are readily damaged during radical prostatectomy, invariably causing impotence.

Erectile function can return, however this may take months or years and capacity often remains poor. Many studies have attempted to improve penile nerve regeneration but have not explored mechanisms underlying the delay in functional recovery.

This is assumed to be due to slow growth of axons, although penile tissues also change following loss of erectile activity.

We have asked whether delayed recovery of the nitrergic nerve-evoked erectile response is due to pre-synaptic (slow axonal growth) or post-synaptic (changes in tissue responsiveness) mechanisms. These components were dissected in vitro following penile nerve injury in adult rats.

Following crush of both penile nerves, excellent regeneration of nitrergic axons occurred after 10–12 weeks but neurogenic relaxation of cavernosum muscle was still relatively poor.

This was at least partly due to attenuated [decreased] tissue responsiveness to nitric oxide (using sodium nitroprusside as a donor) from 3 weeks after injury. Western blotting also revealed a modest reduction of soluble guanylyl cyclase.

A second model of penile nerve injury, unilateral cut, completely denervated one side but retained potential for penile erection. Some anatomical and functional recovery occurred after 9–11 weeks (probably due to sprouting from contralateral uninjured axons), but nitroprusside-evoked relaxations were unaltered from at least 3 weeks onward.

These data suggest that erectile dysfunction following extensive nerve injury may be exacerbated by postsynaptic changes in nitric oxide signaling, even when nerve regeneration occurs.

This may be prevented by continued activation of penile tissues to retain normal perfusion.

I've trolled the boards off and on for the past few months and finally decided to create an account. Every time I start reading about other peoples experiences and the science behind what might be going on in my body I get very depressed which is why I've avoided doing so. I'll search for the proper thread to post my full story but I couldn't agree more with the above quote. Approximately 5 months ago I entered my first sexual relationship in years, and while I had problems maintaining an erection suitable for intercourse the first couple of tries, after about two weeks of trying I was finally able to have satisfactory intercourse.

I found out about this website while searching for people's experiences with Rogaine. My initial thought was that Rogaine HAD to have something to do with my symptoms since I hadn't taken Finasteride in years. I mention this because I stopped using Rogaine during that two week time period and noticed the difference with my ability to achieve and maintain a decent erection. Of course, the quality of the erection I do get is nothing like the erections I used to get prior to using Finasteride.

Anyway, my girlfriend and I were having sex at least 3 days a week and my erections remained consistent. As of late, we have not been having sex as often and I noticed how the potency of my erections and longevity has decreased. My girlfriend is not on birth control therefore I have to use condoms and sometimes it is difficult maintaining an erection long enough to put on a condom and enter my partner. Once I am in though, the erection will last throughout the entirety of intercourse and has only gone flaccid inside her once or twice.

Another thing worth noting is that I am an athletic person. I run and lift weights and have been thinking about supplementing nitric oxide to see what effect it has on me. I don't like most NO supplements since they have too much caffeine so I am trying l-arginine, only in a single 500mg dose/day, but am generally scared of making things worse. I took one 500mg pill two days in a row pre-workout and I generally felt better but had trouble maintaining an erection this weekend.

Finally, the hardest part is finding someone patient enough to allow follies and allowing yourself to get close to someone sexually. My partner was far from ideal but I was stubborn enough to make it work.

So why then every time I work out (weight lifting and running) my dick starts to feel cold and numb. Isn't also dangerous to overextend ourselves being fatigued and all? - I almost feel I should be doing the opposite - lay in bed all day keeping my junk warm (but I'm not since everyone here says go exercise).
